Waiver: I/We the undersigned, request that the following repairs and/or maintenance be done to my/our Unit. I/We request that the said repairs and/or maintenance be attended to as soon as possible from the date of the request. For such purpose, and the work requested below necessitates entering my/our Unit, I hereby give you permission to do so during reasonable daylight hours to effect the repairs. This acknowledgement shall operate as my consent at the time of entry for you to enter my/our Unit in order to effect the requested repairs, notwithstanding my absence from the Unit at the time of such entry and at the time that such repairs are made.
